Tetanus is a disease preventable via toxoid vaccination. Among developed countries, Japan has a high percentage of unvaccinated people in the older generation. Because tetanus antitoxin antibody titers fall below protective levels in approximately 10 years, additional tetanus toxoid should be administered to trauma patients if more than 10 years have passed since previous vaccination. The guideline for tetanus prophylaxis is a recommendation for developed countries with advanced vaccination systems, such as the United States, and its application to Japan is questionable. Since tetanus vaccine became a routine vaccination in Japan in April 1968, it is important to aggressively administer tetanus toxoid and tetanus immune globulin in trauma and burn patients born before April 1968.
