【コラム】ICUのあり方をどう考えるか—ICUの現状と課題についての一考察

What functions should an ICU have?

ICUは医療の質と病院の経済性に甚大な影響を及ぼす治療室であり，急性期病院にとって極めて重要である。平成26年度診療報酬改定において，特定集中治療室管理料1・2の上位加算が新設され，点数が大幅に引き上げられたことが注目されている。しかし，1床当たり20m²以上である面積要件がクリアできない施設も多く，算定できることが必ずしも質の高いICUであるという評価に結びつくとは言い切れない面もある。

　大きな地域差を生じさせることなく，質の高いICUを整備していくことは我が国の医療にとって重要課題である。そのためにはSOFAスコア等を用いて診療報酬で適切な評価を行うことが望ましい。ただ，今後，生産年齢人口は減少し財源にも人財にも制約があるなかでは，施設集約の検討が必要になるだろう。

Intensive Care Units （ICU） are an essential function for acute care hospitals with considerable impact on quality of care and hospital finances. The premium medical fees （i.e. ICU administrative fees 1 and 2） were newly established upon revision of the medical fee table in 2014. The higher fee got attention from hospitals, but most hospitals are not able to meet the space requirement, which should be more than 20㎡ per bed. Therefore, the higher medical fee does not necessarily promote better quality of care in the ICU.

　Appropriate deployment of high-quality ICU care regardless of region is an important issue to provide better critical care for society. It is necessary to evaluate the level of health care services provided in the ICU using valid tools, such as SOFA in terms of medical fees. Furthermore, consolidation of hospitals with ICUs would be considered under the circumstance that the productive-age population and healthcare budget are constrained in the future.
