2024年の働き方改革の本格的な施行に向けて，医師の労務負荷軽減が喫緊の課題となっている。そのための解決策として，医療現場における情報通信技術information communication technology（ICT）の活用が期待されている。米国で普及している急性期の遠隔診療であるTele-ICUがその解決策として期待されている。本コラムでは，本邦にTele-ICUを導入していくうえで，導入による効果と弊害，システム概要，海外での事例紹介，導入における課題，COVID-19の環境下におけるtelemedicineの可能性などについて解説する。

For the full-scale implementation of work-style reforms for doctors in 2024, reducing the workload is an urgent issue. However efficient solutions for this problem have not been prepared. As one solution, utilization of Information Communication Technology （ICT） in medical practice is expected for solve some of these problems. The Tele-Intensive Care Unit （ICU）, which is telemedicine for the care of acutely ill patients, now prevailing in the United States, is expected to be a solution. In this column, in introducing the Tele-ICU in Japan, the effects and adverse effects of Tele-ICU introduction, a Tele-ICU system overview, overseas case introduction, introduction challenges, and the COVID-19 environment are discussed to explore the possibilities of Tele-medicine.
