緩和ケアは生命を脅かす疾患をもつ患者とその家族に対して，苦痛を取り除くことによって生活の質を改善することを目的としたアプローチであり，多職種のチームによって提供されなければならない。臓器不全により移植待機している患者，もしくは移植後の患者は緩和ケアの恩恵を享受すべきであるにもかかわらず，実際は緩和ケアにアクセスできていない。本稿では緩和ケアの利点，緩和ケア導入における障壁と，その解決方法の可能性に関して論じる。

Palliative care improves the the quality of life of patients and their families facing problems associated with life-threatening illnesses. It should be provided by an interdisciplinary team, which is comprised not only of physicians, but also includes nurses, social workers, chaplains, etc. While pre- or post- transplant patients with organ failure could benefit from palliative care, it has not been provided to the level that it should be. This article will discuss the benefits of palliative care, barriers to incorporating palliative care of transplant patients, and possible solutions to these problems.
