This article reviews preventive interventions for environmental aspects of the intensive care unit (ICU) to improve cognitive impairment and psychiatric disorders in patients with postintensive care syndrome (PICS) and families with PICS (PICS-F). Earplugs and noise cancelling headphones worn by critically ill patients are associated with a significantly decreased incidence of delirium as a risk factor for PICS, although the efficacy of sleep promotion with these interventions in the ICU is still unclear, probably due low patient compliance with these noise cancelling devices. Posttraumatic stress disorder in ICU survivors may be avoidable using ICU diaries, which can provide an accurate and informative collection of events during the ICU stay and improve memory recall, although these memories may be unpleasant for patients, and encourage the healing process with debriefing for traumatic events. Close communication with ICU medical staff is critical to prevent PICS-F. This allows family members to understand a patient's clinical course and enhances family participation in patient care including writing diaries, which may be effective to reduce negative emotions. Early interventions by a multidisciplinary medical team are reported to be useful. All of these interventions seem promising to prevent PICS and PICS-F, although further clinical investigations are needed.
