耳栓骨導檢査法の研究

　Rinne検査，Gellé検査及びFowlerの音の大さ比較検査法は鑑別診断に用いられている。然しこれらの有名な検査法は臨床に応用した場合に正確を欠ぎ且つ不便なことがある。その上これらの検査法の原理が未だ明瞭に解明されていない欠点がある。特にGellé検査は中耳障碍を検出する方法として興味を感じたが，原法の通り行えば雑音を伴うことなしに外耳道に気圧を与えることは困難である。変法であるMacfarlan法も同樣な欠点がある。

　Gellé法に於ける外耳道加圧が骨導低下を来す機構を研究中に耳栓骨導検査に於ける骨導増加がGellé法の骨導低下と同一現象であることを発見した。耳栓骨導の感度増加の現象はPohlman並にKranz，Bekésy，MacNally並にErickson，Kelley並にReger，Aubry，Causse並にChavanne，Lierle並にReger，Sullivan，Gotlied並にHodges等によつて既に報告されており，早いものは約20年前である。然るに欧米の最近の聽力検査法中には耳栓骨導検査が殆んど採用されていないところを見ると本法は今日でも実用化されていないようである。そのため耳機骨導検査は私が初めて発見したと思つた程である。本法の一般化が妨げられた理由は耳栓骨導増加が如何なる機構によるものか全く解明されておらず且つ如何なる報告を見ても単に耳栓と述べているだけで耳栓の構造を明瞭に述べていないし，（尤もSullivanとその協同者は水或はグリセリンに浸した綿球と述べているが，実用上不便であり）或報告では骨導検査音の種類が多過ぎて不便であつたりしたためであろう。

　NCHI presents a method of making a bone conduction test, which he calls the blocked bone conduction test, by using plastic plug in which a hole 1mm in diameter is bored through its long axis for blocking the external canal. Two test tones are used, 250 or 256 and 800 c. p. s The blocked bone conduction test is adequate only in sofar as detecting conduction impairments; Fowler's and difference limen tests only for nerve impairments. By combining these two tests diffential diagnosis of central deafness is made possible. In addition, in studying the mechanism of Gelle's test, the author finds that the increased loudness which is encountered in the bone conduction when plastic plug external canal block is used appears to be the same phenomenon which is met with as the decreased loudness in the former but, opposite in effect.
