患者の苦痛表情分析による緊急性評価法—ビデオメールを用いた遠隔看護システムの構築

Evaluation of Priority of Video Mail in Telenursing System: Using Extraction of Facial Expression of Patient's Pain

緒言

　「遠隔看護」という言葉は，「遠隔医療」という言葉に倣った筆者らの造語である。したがって，言葉としての市民権はまだ得ていないのが現状である。一般的には，「看護」と「遠隔」は相反する言葉のように捉えられがちで，その逆説的なところに猜疑的な戸惑いを感じられるかもしれない。しかし，世界中でもまれな日本の超高齢社会化現象や，病院医療におけるコスト負担の増大などを考える時，在宅での医療や看護の必要性は増大すると予測される。また，通院の必要性が高くなく，医師や看護者の直接的な処置を得なくても，それらの指導や指示によって患者自身での自立的なケアが可能なケースもある。このような場合，「看護」と「遠隔」は必ずしも相反するものではない。自立的な患者もしくはその介護者と看護者が物理的空間において遠隔であっても，看護に必要な情報の双方向における伝達を確保することで看護的空問（これも筆者らの造語であるがここでは主に心理的空間を指す）における遠隔を取り除くことは可能であると考える。

　看護には，看護専門職による広い意味での医療技術や知識を活用して看護に携わり，患者とのメンタルな部分を含んだコミュニケーションの構築が求められている。本焦点では，このような看護専門の領域を生かすため．その陰でのバックアップを図る技術として遠隔看護システムを提案する。遠隔看護では，患者が直接的な医療処置を必要としない期間の看護を対象としている。

　This study has examined the extraction of a patient's facial expression of pain through the use of a video-mail system, in order to evaluate the priority of the video mail in the tele-nursing system. The patient's appearance is recorded by a video camera. Next.consecutive video pictures are resolved into still and individual frame images. The face area and background are extracted by calculating the difference of the consecutive frames. The position of the pupils is extracted from the face area by using the area separation filter. The position of other face parts necessary for recognition of the expression is also extracted. In addition, the area of the middle of the forehead is extracted. This paper proposes that the wrinkle in the middle of the forehead is an important element in the recognition of it facial expression of pain.
