　成人先天性心疾患患者がキャリーオーバーを経て疾患に対する認識を変化させていくプロセスを明らかにすることを目的に，17名の成人先天性心疾患患者に半構造化面接を行ない，grounded theory approachの手法を用いて分析を行なった。その結果，〈疾患の常態化〉の段階，《疾患との直面》を経験する段階，〈疾患をもつ生の意味づけ〉を行なう段階の3段階のプロセスが抽出された。

　〈疾患の常態化〉の段階において対象者は，疾患重症度によらず，疾患は自分の身体的特徴であり，自分にとっては〔疾患があることが普通〕のことだと認識していた。

　《疾患との直面》を経験する段階において対象者は，病状の変化などを契機とした〈死の意識から生の志向への転換〉，就職，結婚などを契機とした〈社会における自己の限界の見極め〉，〈親からの自立への模索〉を通して，〔疾患をもつ自分に気づき，向き合う〕という《疾患との直面》を経験していた。この経験は対象者が疾患に対する認識を変化させていくプロセスにおいてターニングポイントとなっていた。疾患重症度の低い対象者や早期の根治術後状態の安定している対象者のなかには，《疾患との直面》を経験していない者もおり，対象者が《疾患との直面》を経験しているか，何を契機として《疾患との直面》を経験したかによって，疾患に対する認識には個人差が生じていた。

　《疾患との直面》を経験した対象者は〈疾患をもつ生の意味づけ〉の段階において，〔疾患があったからこそ〕と疾患をもつ生を肯定的に意味づけるようになっていた。

　［Background］ As the number of adults with congenital heart disease increases, there has been a growing demand for the uninterrupted management of congenital heart disease from childhood to adulthood. However, little is known about how adult congenital heart disease patients deal with the difficulties in their life and how it influences their appreciation of the illness to experience these difficulties. The present study was performed in order to investigate the process of change in the patien's appreciation of congenital heart disease.

　［Methods］ A semi-structured interview was conducted on 17 adults with congenital heart disease, and interview data were analyzed in accordance with the grounded theory approach.

　［Results］ The age of the participants ranged from 22 to 35 years. The primary diseases and operations were radical surgery for tetralogy of Fallot, surgical closure of ventricular septal defects, and the Fontan operation. The analysis revealed three stages of change in the patients' appreciation of the illness during transition to adulthood: (1) having illness as one's ordinary state, (2) confronting the illness, and (3) finding meaning in life with the illness. Confronting the illness was found to be a turning point during the process of change in appreciation of the illness by adults with congenital heart disease.

　［Conclusions］ Through the transition to adulthood, patients with congenital heart disease confronted their illness, which they had thought to be their ordinary state in their childhood.
