めまいは平衡障害の一分症であり，身体平衡に関係する神経系の障害で起こる．多くは末梢性めまいであるが，生命予後への影響や重篤な神経系機能障害をきたす中枢性めまいに注意を払い診療に当たるべきである．めまいの診療において，問診でめまいの性状・背景因子などを適確かつ迅速に把握すること，随伴症状のチェック，眼振や体平衡検査による前庭機能ならびに平衡機能検査，画像検査による病巣診断などが診断の鍵となる．中枢性めまいの鑑別が重要となり，脳神経症状，運動・知覚障害に注意を要する．治療は，急性期のめまいや悪心・嘔吐に対する対症療法，前庭機能不均衡を是正し前庭機能の早期回復をはかる治療，耳石置換法などの理学療法に大別される．病態・病像にあった治療の選択が必要となる．以上，めまいの診断ならびに治療の実際をまとめた．

Vertigo is a symptom of disorders of equilibrium.　Most causes of vertigo are caused by some problem with the peripheral vestibular apparatus, but it is necessary to pay attention also to causes, prognosis and severe central neurological disorder.　Appropriate management of vertigo requires the correct diagnosis.　The background of vertigo, the accompanying symptoms, nystagmus, the equilibrium test, and diagnostic imaging (Radiography) holds the key to the diagnosis of vertigo.　Differential diagnosis of the central vertigo becomes very important, which needs attention to check symptoms of the central nervous system, motor, and sensory disturbance.　The treatment of vertigo may reduce symptoms of vertigo, nausea, vomiting, and promote early recovery of vestibular dysfunction.
