副咽頭間隙atypical lipomaの1例

Atypical Lipoma of the Parapharyngeal Space

I．はじめに

　脂肪肉腫は軟部悪性腫瘍の約10％を占め^1）比転的頻度は高いが，副咽頭間隙に発生する悪性の脂肪性腫瘍はきわめて稀である^2〜4）。

　われわれは30年もの長い経過中に再発を繰り返した副咽頭間隙のatypical lipoma（well differentiated lipoma-like liposarcoma）の1例を経験したので報告し，若干の文献的考察を行った。

　A 70-year-old woman who was previously diagnosed as having lipoma or liposarcoma and recurred five times after excision in the last 30 years is reported.

　Histologically, the tumor was diagnosed as lipoma until the fourth excision was done and diagnosed as well differentiated liposarcoma during the fifth and sixth excision.

　However, the tumor was found to have the same histological findings when reexamined in detail.According to the WHO classification, it was classified as well differentiated lipoma-like liposarcoma, although a decision was made to classify the tumor as atypical lipoma according to the classification proposed by Kindblom and Evans.

　In the sixth operation, a parapharyngeal tumor was excised by the transcervical approach with mandibulotomy.
