Ⅰ．はじめに
小児の副咽頭間隙膿瘍は非常に稀である。Brianら1）は93例の小児頸部膿瘍を報告しているが，副咽頭間隙膿瘍は3.4％，3例のみであった。それと同時にこれらの感染症はときに急激な増悪をきたし，浮腫や腫脹による上気道閉塞や縦隔へ膿瘍が進展して重篤な病態を引き起こすことがある。そのため，治療に当たっては迅速かつ的確な診断に基づいた適切な外科的治療が重要となる。今回われわれは4歳・男児の副咽頭間隙膿瘍症例を経験し，頸部外切開により治療し得たのでこれを報告する。
Parapharyngeal infections in the pediatric cases are very rare. We encountered a 4 years and 6 months old male patient with deep neck infection originated in the parapharyngeal space. The patient didn't respond to antibiotic therapy in a local hospital. Contrast-enhanced CT showed a 20×25mm low attenuation mass with ring enhancement in the left side of the retropharyngeal space. Surgical drainage externally through a submandibular incision, followed by antibiotic administration. Thenafter he was recovered about one week after the operation.
Early open surgical drainage is recommended in severe cases with no response to antibiotic therapy.
