 This paper describes a modified Hirano technique for glottic reconstruction following hemilaryngectomy and its early functional results. The technique adopts the use of a superiorbased sternohyoid muscle flap and an island cervical skin flap and preservation of a bridge-like segment of the thyroid ala on the affected side. Nine patients underwent this procedure. Their post-operative function was evaluated. The phonatory function was better when the margin of the island skin flap was sutured to the middle of the posterior wall of the larynx than when it was sutured to the lateral wall of the posterior glottis. In the latter cases, there was a marked glottal incompetence during phonation. The glottal incompetence could be corrected by means of a secondary mediofixation procedure.
