多発性脳神経症状を呈したsarcoidosisの1例—sarcoidosisの神経症状

A Case of Sarcoidosis with Multiple Neuropathy; Neurosarcoidosis

I．はじめに

　系統的に全身に非乾酪性類上皮性肉芽腫を形成する疾患はサルコイドージスsarcoidosis（以下サ症）と呼ばれる。その疾患の病態はいまだ不明な部分も多いが，本邦では厚生省が特定する難治性疾患に指定されてサ症診断における新しい知見や診断法の進歩ないし改善があり，疾患の輪郭も徐徐にではあるが明らかになりつつある。研究班の定めたサ症判定基準によればサ症の診断は組織学的に壊死をほとんど伴わない類上皮細胞結節を証明することであり，組織学的診断が下されない場合は臨床症状から診断されるとある。

　サ症の臨床症状は多くの臓器にわたるためきわめて多彩であるが，主要病変は肺，リンパ節，皮膚，眼，神経，筋，肝，脾，耳下腺，骨にある。肺では両側の肺門リンパ節腫大（BHL），肺野病変，眼ではブドウ膜炎，皮膚では結節性紅斑，耳下腺では耳下腺炎の出現が特徴的で，二つの臓器以上に症状を認めたときサ症の診断は確定する。

　A 33-year-old male with sarcoidosis presenting ipsilateral or bilateral V, VI, VII, IX, X cranial nerve palsy was reported. He was suffering from disturbance of the ocular movement of both eyes and trigemia on the left side, and later on, bilateral facial nerve palsy, difficulty swallowing, and hoarseness developing during a 3-months period. There were no significant symptoms except neurological manifestations, and skin nodules which dispersed in the body. Clinical diagnosis was established from the diversified cranial neuropathy with different phases, the multiple skin nodules, and the elevated angiotensin enzyme. After steroid therapy the patient was free of all symptoms except facial nerve palsy.
