当科における10年間の扁摘の統計学的観察

A 10-Year-Follow-Up Study on 780 Tonsillectomy Cases

I．はじめに

　口蓋扁桃摘出術の適応に関して今日なお多くの論議のあるところである。そこで当科教室で昭和50年6月より昭和59年3月までの10年間に扁桃摘出術を施行された症例について，疾患，年齢などについて検索ならびにアンケート調査を行ったのでその結果を報告する。

　当科における扁桃摘出術（以下扁滴と略す）の適応の診断基準は表1^1）に示されているごとく手術が施行されている。

　780 cases of tonsillectomy which were performed for the last 10 years were followed up and clinical data regarding their diseases, age, tonsillar shapes, answers to questionnaires given to the patients were analysed.

　The results were as follows :

　1. Prevalent rate of recurrent tonsillitis and palatine tonsillar hypertrophy were the highest in 5-10 years of age, but the age distribution of chronic tonsillitis was broadly scattered.

　2. The positivity of ASO-titer had no relation to each tonsillar disease.

　3. The tonsillar shape in the majority of cases showed a round-shape or lobulated-shape.

　4. Most of tonsillar hypertrophy were associated with adenoidal hypertrophy and otitis media with effusion.

　5. Subjective symptoms, such as recurrent fever, low grade fever and pharyngeal pain were effectively improved after tonsillectomy.

　6. In more than 50% of the cases with chronic sinusitis or nasal allergy, the subjective symptoms were improved.
