上顎洞アスペルギルス症の術前細胞学的診断

Preoperative Diagnosis of Fungus Infection of Maxillary Sinus: A Cytological Approach

I．緒言

　片側に頻発する頬部腫脹，鼻閉，鼻汁過多あるいは血性鼻汁等の臨床症状を示す副鼻腔真菌症，特に上顎洞真菌症は，X線所見，CT scan等によっても腫瘍性病変との鑑別がしばしば困難である。ほとんどの場合，試験開放による生検，または副鼻腔根治手術の際に得られた組織片から病理学的に診断される。

　上顎洞真菌症に対する診断の一助としての洞内洗浄液の細胞学的検索は1933年Stevenson^1）以来，2,3の報告を見るのみで^2,3），さほど臨床的にも用いられず現在に到っている。我々は副鼻腔真菌症の疑いのある症例に対して数年来，上顎洞洗浄液の細胞学的検索を行ってきた。ここにいささかの知見を得たので，2症例を紹介し，併せてその有用性と今後の検索上の問題等について述べたい。

　Preoperative diagnosis of fungus infection of the maxillary sinus is often difficult, particularly in differentiating it from neoplasm. Therefore, an attempt to introduce a cytological approach using aspirated materials from the sinus was made.

　Case 1 : coexistence of squamous cell carcinoma with aspergillus was proved with a conventional Papanicolaou staining ; the coexistence of fungus and malignancy in the sinus is the first case ever reported in the literature.

　Case 2 : infection with aspergillus was determined using cell-block specimens, i.e., formalinfixed and paraffin-embedded cellular constituents aspirated from the sinus.
