耳鼻咽喉科領域悪性腫瘍の細胞学的検索

CYTOLOGICAL INVESTIGATION OF OTORHINOLARYNGOLOGICAL MALIGNANCIES

　I．はじめに

　耳鼻咽喉科領域の悪性腫瘍に関する細胞学的診断の報告は，すでに1949年Morrison^1）を始めとしてFriedmann^2）〜4）らにより発表されている。一方口腔内では1951年渡辺^5）が口腔外科総会にて発表，同年Montgomeryら^6）により始めて記載されている。特に上顎洞の細胞診に関してはFitz-Hugh^7）〜10）らが検査手技の比較的簡単なこと，手術的侵襲を加えることなくまた被検者にはさしたる苦痛を与えず何回でも実施でき，さらに患者にとつては腫瘍細胞の播種の危険性が少ないこと，診断の適中率も非常に高いことなどを述べている。

　今回われわれは耳鼻咽喉科領域の悪性腫瘍の早期発見とスクリーニングを意図して細胞診を実施して来たところ，口腔内に発生した腺癌と上顎洞原発と思われる特徴ある細胞像を示す悪性黒色腫を細胞学的に診断し，その結果術前に綿密な治療計画の立案実施が可能となり，現在局所の再発や隣接リンパ節転移，全身への遠隔転移も認められず良好な経過を辿つている症例を経験したので報告する。また耳鼻咽喉科領域の細胞診について，特に口腔，上顎洞の細胞診を中心に若干の考察を加えた。

　Cytological investigation was successful in detecting cases of oral adenocarcinoma and malignant melanoma in the maxillary sinus. Of thesecases, cytodiagnosis, characteristic cell behaviors and treatment were studied and reported. Some observations were also made regarding to cytodiagnostic approaches in the field of otorhinolaryngology.

　It was further proved that the cytological approach could be valuable as screening for early detection of malignant tumors as well as in surveillance of postoperative recurrence.
