慢性扁桃炎に合併した壊死性リンパ節炎

A Case of Chronic Tonsillitis Accompanied with Necrotizing Lymphadenitis

　I．はじめに

　1972年，藤本ら^1）は，頸腺結核症を除外する目的で行った頸部の摘出リンパ節の中に，necrotizing lymphadenitisとの病理診断を受けた一群があり，これらが臨床的にも似かよった病態像を示すことに着目して，新しい疾患概念として提唱した。これとほぼ同時に菊地ら^2），若狭ら^3,4）によっても同様の病変が報告され，その原因，病理発生解明に，にわかに関心が寄せられるようになった。

　今回われわれは，扁桃炎をくりかえし，頸部リンパ節腫脹があるため口蓋扁桃摘出術を施行した若い女性で，術後も頻回に発熱をくりかえすため試切した頸部リンパ節に，壊死性リンパ節炎の病理診断を得た症例を経験した。ここにその臨床像を紹介し，摘出扁桃と頸部リンパ節の病理組織学的所見について比較検討すると同時に，壊死性リンパ節炎についての知見を紹介し，若干の文献的考察を行った。

　A young girl patient was found having necrotizing lymphadenitis in the neck, which was confirmed after her tonsils were removed because of chronic tonsillitis. The diagnosis of chronic tonsillitis was made by not only her course of present illness but laboratory data and past history. Because of recurrent fever even after tonsillectomy, a cervical lymph node was biopsied in order to rule out tuberculous lymphadenitis. The histological findings showed necrotizining lymphadenitis and suggested that her symptoms came from it.

　Discussion was made on the recent knowledges about this disease and characteristics of these clinical and pathological findings.
