低音域のみの突発的感音難聴症例の検討

A Study on Low-tone Deafness with Sudden Onset

　I．はじめに

　突発性難聴は，突発的に発症する原因不明の感音難聴の総称である^1）。一般に，その難聴は，中等度から高度であり，オージオグラム上の型から，高音漸傾型，高音急墜型，水平型，山型，谷型，低音障害型，ろう型などの難聴型に分類されている^2〜4）。しかし，このうち，低音障害型の報告は少なく，新鮮例を検討した太田ら^3）は，30例中5例に，同じく，佐藤^4）は，44例の新鮮例中1例に低音障害型を観察しているにすぎない。さらに，突発的な低音障害型感音難聴であって，しかも，中，高音域の聴力が正常であるような症例の報告は，われわれの検討し得た範囲では見当たらない。

　今回，われわれは，耳閉感を主訴に来院した症例に，低音域のみの感音難聴を観察し，突発性難聴に準じ治療を行ったところ，早期に聴力の改善をみた6症例を経験したので，症例の分析・検討を行い，若干の考察を加えたので報告する。

　Six cases showing low-tone sensory-neural hearing loss with sudden onset were reported. Pure tone audiometry revealed that the sensoryneural hearing loss was much severe in lower frequency tones. All but one patients, showed normal hearing above 1000 Hz, and the one exhibited normal hearing above 2000 Hz.

　All patients complained fullness of the ear and one patient complained of mild hearing loss, but five patients did not feel deafness. Steroid hormone (predonine) was given to all patients, and hearing loss recovered to normal shortly after treatment.

　From the analysis of the results, we diagnosed these cases as idiopathic sudden deafness with localized lesions in the cochlea.
