インシュリン局所治療が奏効した悪性外耳炎の1例

Local Administration of Insulin in a Case of Malignant External Otitis

　I．はじめに

　糖尿病に併発する感染症は時に難治性で，治療に難渋することがあるのは周知の通りである。われわれは，強力な化学療法と手術療法によっても治癒傾向を示さない悪性外耳炎，すなわち糖尿病に併発した緑膿菌性の広範囲の外耳炎，乳突炎の1例に対して，insulin局所療法を試みて，顕著な効果を認めたので報告し，本療法の実際と作用機序について解説を加えたい。

　A 61-year-old man with malignant external otitis was treated by a large dose of antibiotics and repeated mastoidectomies in conjunction with systemic insulin. Since no noticeable effect was obtained by these treatments, we attempted local administration of insulin. Being encouraged by the initial good response, intra-aural injection of insulin was continued for two months. The infection was completely controlled. Reviewing therapeutic mechanisms of this treatment, we concluded the local administration of insulin to be valuable for treatment of malignant external otitis.
