I.はじめに
筆者らは先に手術室においてだけでなく,外来耳科診療においてもマイクロサージェリーの手技や器具を導入すれば便利なことが多く,診療の質も向上すると主張した1)。その応用の1つとして,以下に新鮮外傷性鼓膜穿孔(鼓膜破裂)の治療法として筆者らが試みた方法を述べる。
耳を叩かれたり,爆発,異物などにより鼓膜に裂傷を生じた時,わが国および欧米における教科書的な従来の治療方針は,いわゆるwatch andwaitとして清潔を保って感染の予防につとめ,ことに耳に水を入れないよう指導し,抗生剤投与を行ったりして穿孔の自然閉鎖を待つことが原則である。かくして3週から3カ月たっても閉鎖しない時は穿孔縁の焼灼とか薄い紙を貼ったりするなど種々の保存的治療が行われている。さらに閉鎖しないときや極めて大きな穿孔の時は観血的に鼓膜形成が行われる。
Ten cases of traumatic tympanic membrane rupture in early stage were treated by the following technique: Using an operating microscope and otomicrosurgical instruments, everted or inverted flap remnants of the perforated tympanic membrane were elevated and were placed back in position. The perforations of all cases treated were closed 5~44 days (23.1 days in average) after repair, while those of 12 untreated cases including 3 permanent perforations needed 10~212 days (60.3 days in average) until closure of the perforations. It is, therefore, evident that our simple technique significantly shortened the time for closure. This method is recommended as a first choice.
