I.はじめに
聴覚失認(auditory agnosia)というのは,あらゆる音の弁別,認知のできない状態で,このうち言語音の了解のみが全く不能な場合を純粋語聾(pure word deafness)という。この「純粋」というのは,内言語の障害がなく,単に言語音の了解だけが不可能という意味である。また,音楽的な認知が不可能になったものを失音楽症(amusia)と呼ぶ。この純粋語聾と失音楽症を含めて,聴覚失認という1)。
側頭葉に聴覚中枢の存在することはすでに知られた事実であり,一側が損傷されても聴力にはほとんど影響を与えないが,両側の場合は,聴覚機能が著しく障害される2)。
A 38-year-old man developed auditory agnosia after surgery for aneurysms in both middle cerebral arteries. CT scan showed low density occupied in the temporal and parietal areas bilaterally.
Audiological results were as follows ; 1) Pure tone audiometry revealed a bilateral mild hearing loss, but the sound localization ability was unimpaired. 2) He showed the marked inability to recognize either simple speech sounds or words. The perception of rhythm and melody was also markedly impaired. 3) The left ear stimulation showed consistently poorer responses than the right ear on tasks involving stapedius reflex, auditory brainstem response and middle latency response. The slow vertex responses were very poor bilaterally.
