外傷性鼓膜穿孔と耳閉感

Sensation of Ear Fullness in Traumatic Perforation of the Ear Drum

　はじめに

　耳閉感という感覚は疾病の部位によってその表現に多少の違いがあると考えられるが，つまったような，水が入ったような，膜が張ったような，山に登ったときのような感じなどの表現がなされる。

　外傷性鼓膜穿孔という突然の穿孔でも患側の耳閉感が起こることは良く知られているし当然のことのように思えるが，患側に耳閉感を訴えず，何ら障害を受けていない対側，つまり健側の耳閉感を主訴に来院する例が存在することを経験し，このような耳閉感の発現がどの程度にみられるかを調査，検討したので報告する。

In 80 cases, ranging in age from 10 to 74, with traumatic unilateral perforation of the ear drum, 35 (43.7%) patients complained of ear fullness in the affected side and 10 ( 12.5%) complained in both sides and 15 (18.8%) complained of ear fullness in the unaffected ear. The cases with ear fullness in the unaffected sides showed normal hearing acuity and A type on tympanogram and they revealed smaller traumatic perforation with a slight hearing loss of the affected ears than those with ear fullness in the affected ear or both ears. These sensation was immediately resolved after the perforation was patched or closed spontaneously.
