副鼻腔炎に対する造影剤の現況—リピオドール・ウルトラフルイドを中心に

Usefullness of Contrast Medium on the Diagnosis of Sinusitis: With Reference to Lipiodol Ultra Fluide

　I．はじめに

　昭和54年に仙台で開催された日本鼻副鼻腔学会で造影剤に関する演題がとりあげられたのは，これまでしばしば用いられてきたモルヨドールが製造中止になったことがきっかけとなったと聞いている。わが国で慢性副鼻腔炎の診断にこのモルヨドールが用いられるようになったのは，戦後に石川・大藤，権田・高橋らの基礎と臨床の両面にわたる研究に端を発していると思われる。しかし上顎洞の病態を造影剤を用いて観察するという点では，すでに1902年久保（猪）が硫酸バリウムを用いて報告したのが始まりのようである。

　造影剤にも種々あり，またこれが副鼻腔炎の診断に応用される際に，どれが最も適当であるかは過去の文献にも述べられてはいるが，その後の報告からみると，やはりモルヨドールが使用されている頻度が高い。しかしその製品が得られなくなった現在，これに代わるものとしてはどれが適当であるかの結論ば得られていないのであるが，過去の文献を参照しながら，リピオドール・ウルトラフルイド（以下リピオドールUFと略）を中心に学会報告の概略と私共の検索成績を含めて述べてみたいと思う。

　In 1930s the reports concerning introduction of contrast media into the paranasal sinus were actively published in the Western countries. However, such reports are rarely published at the recent literature, and the reason of this is probably due to CT-scan and tomography which have clinically become available.

　In our country, a contrast medium. Moljodol, had been used since 1945, but recently it is impossible to use this material because the manufacturer discontinued to produce it. Therefore, various types of contrast media have been examined to find the alternatives for Moljodol, and Lipiodol Ultra Fluide was recommended by many investigators. This material is characterized by high content of iodine, low viscosity and easiness of injection into the maxillary sinus. On the other hand it had a disadvantage that leakage from the maxillary sinus may easily occur with it.

　The diagnosis of maxillary sinusitis utilizing contrast media is useful to know not only the outline of the walls of sinus also the functions of the mucous membrane, and further studies on this subject are warranted.
