いわゆる急性中耳炎性顔面神経麻痺—とくにその成因と治療について

Facial Palsy Due to Acute Otitis Media

　I．はじめに

　急性中耳炎の合併症としての顔面神経麻痺は，抗生剤の出現以後減少した^1,2）。これは抗生剤によって急性中耳炎が早期に治療されるためと思われる。しかし，抗生剤によって中耳炎が予防されることはなく，さらに最近では反復性・難治性中耳炎が増加する傾向にあり^3），これに伴って急性中耳炎性顔面神経麻痺の増加の可能性もあると推測される。

　多くの場合，抗生剤の投与により急性中耳炎そのものはよくコントロールされるが，顔面神経麻痺が生じた場合には，保存療法だけでは麻痺の回復は困難なこともあり，いたずらに抗生剤治療に頼っていては顔面神経麻痺の治療の機会を逸することになる。

Facial palsy due to otitis media has become rare since the advent of antibiotics. However, not all otitic palsies can be cured by antibiotics alone. Eight cases with facial palsy due to acute otitis media were reported, and pathophysiology and treatment of this disease were discussed.

From our observation of these cases, the following conclusions were made: (1) facial palsy, generally considered due to acute otitis media, is, in reality, a complication of mastoiditis, (2) since treatment by antibiotics alone appeared less effective for quick recovery in cases of acute otitis media followed by facial palsy, mastoidectomy at appropriate time is definitely recommended.
