副鼻腔嚢腫の臨床と表面微細構造

Cystic Lesion of Paranasal Sinuses: Clinicopatho-logical and Scanning Electron Microscopic Observation

　I．はじめに

　上顎部の嚢腫性疾患とくに術後性頬部嚢腫に関しての報告は少なくないが，副鼻腔嚢腫全般について，臨床的ならびに超微細構造的観点から解析した報告はほとんどない^1,2,3）。今回筆者らは当教室にて昭和52年11月より昭和55年2月までの間に入院手術した副鼻腔嚢腫について，臨床統計的観察を行うとともに，一部症例について走査電顕（SEM）的観察を行った。その結果，眼底所見および表面微細構造の上で若干の興味ある知見を得たので，ここに報告する。

　Twenty-seven cases of cystic lesions of the paranasal sinuses treated at Kagoshima University Hospital from November, 1977 to February, 1980 were investigated statistically and clinicopathologically. In some cases funduscopic observations were attempted and revealed the choroidal folds of the fundus of the eyes in 5 cases out of 8 patients who presented with exophthalmus, and location of the cysts seemed to be related to the distribution of the choroidal folds in the eye ball. By scanning electron microscopic observation of the inner surface of the cysts, there was the definite correlation between the SEM findings and content of the cysts. From the clinicopathological viewpoint, significance of the choroidal folds and the ultrastructural findings in the cystic lesions in the paranasal sinuses were discussed.
