　I．緒言

　悪性黒色腫の治療は，現在は手術療法が主体であるが，放射線療法，化学療法，免疫療法にも著しい進歩がみられるようになつてきた^l）〜3）。特にBCGを使つた免疫療法は注目を集めており，全身転移を来たした悪性黒色腫症例に対して，BCGの腫瘍内注射により著効を得た報告もみられる^4）5）6）。

　われわれは下鼻甲介に原発した悪性黒色腫症例に対して，BCGの腫瘍内注射を行なつて腫瘍の著明な縮小を来たさしめた後，残存腫瘍を手術的に摘出し，良好な経過を辿つている症例を経験した。本例において，腫瘍組織のBCG投与による変化を光顕的および電顕的に観察し，興味ある知見を得たので，この所見に基づき耳鼻科領域の悪性黒色腫の治療，特にBCGによる免疫療法について若干の考察を加えてみたい。

　A patient with malignant melanoma in the nasal cavity was treated by BCG immunotherapy with subsequent surgery. The direct injection of BCG solution into the melanoma tissue resulted in a marked regression of the tumor within 40 days, however the tumor began to grow 3 months later and filled the nasal cavity in the next 2 months.

　Histology of the tumor on the 10th day after the initiation of BCG therapy showed noticeable infiltration of monocytes and necrosis of the tumor. Ultrastructural findings disclosed that the fusion of melanoma cells and monocytes, and phagocytic action of monocytes. It was concluded that malignant melanoma should first be treated by BCG immunotherapy before surgical treatment.
