鼻副鼻腔軟骨腫2症例

TWO CASES OF NASAL CHONDROMA

　I．緒言

　鼻副鼻腔領域における軟骨腫は，初発症状に乏しく緩慢な発育により徐々に増大し，鼻閉を主訴とすることが多い。しかし，その発生部位の特異性から，周辺諸器官特に視器に対する障害が発現して始めて受診する症例もある。われわれは鼻中隔原発および上咽頭から鼻腔に広く進展した軟骨腫各1例を経験したので若干の文献的考察も加えてここに報告する。

　Two cases of nasal chondromas are reported.

　Case 1. A girl, aged 10, complained of a visual disturbance of the left eye and headache. Anterior nasal speculum examination and tomography of the face revealed a large tumor in the nasal cavity as well as in the paranasal sinuses. The tumor was removed surgically through transantral approach under general anesthesia. It weighed 48 grams and histopathologically it was diagnosed as a chondroma which originated from the nasal septum. Postoperatively the patient made a slight recovery of the vision but her other symptoms completely disappeared.

　Case 2. A woman, aged 60, complained of bilateral nasal obstruction. Examination revealed a large tumor in both nasal cavities and the palate which was found to be swollen and hard to touch. The growth was removed surgically through trans-palatal approach under general anesthesia. But, at this initial operation only about 32 grams of the tumor was removed. Histologically the diagnosis was a chondroma.

　Six years later the growth recurred. Through similar approach as the initial one the tumor was completely removed. The patient made a satisfactory recovery.
