鼻腔，口腔悪性黒色腫の4症例

MALIGNANT MELANOMA OF THE NOSE AND MOUTH, A REPORT OF FOUR CASES

　I．はじめに

　鼻腔，副鼻腔および口腔内に発生する悪性黒色腫の報告は少なく比較的稀な疾患である。鼻腔例の最初の報告は1869年Lücke^19）により，また口腔例の最初の報告は1859年Weber^26）によりなされた。本邦では中村ら^32）によると鼻腔例64例口腔例66例が，また最近10年間では鼻・副鼻腔例38例が報告されているにすぎない^29）。われわれは最近1年間に3例の鼻腔内悪性黒色腫と1例の硬口蓋に発生した悪性黒色腫を経験したので若干の文献的考察を併せてここに報告する。

　Malignant melanoma of the mouth and nose is relatively rare in occurrence.

　Clinical and histological findings of 3 case of malignant melanoma of the nose and one, in the hard palate are reported. One of the three melanomas of the nose was an amelanotic melanoma originating from the nasal septum. One of these cases showed metastasis in the neck and lungs. The other 3 cases have shown no sign of recurrence up to the present

　In the case of malignant melanoma of the hard palate, the neck metastasis was already manifested at the time of operation.

　Chief complaints were nasal obstruction and epistaxis in all cases of nasal involvements.

　Surgical therapy was the choice of treatment in all cases. Irradiation therapy was combined with surgery for those cases with metastasis.

　Bleomycin was administered in one case with metastasis, but it could not be continued due to severe side effects.

　Biopsy in these cases are contraindicated becuase of the danger of rapid progress of the tumor.
