Wegener肉芽腫およびその類似鼻疾患（壊疽性鼻炎）の発症部位と進展様式に関する検討

A STUDY ON THE SITE OF ORIGIN AND PROGRESS OF WEGENER'S GRANULOMATOSIS AND RELATED NASAL DISEASES (NECROTIC RHINITIS)

　I．はじめに

　壊疽性鼻炎は鼻腔を中心に壊死性肉芽性変化をきたす予後不良の疾患群に対して用いられる病名であるが，近年Wegener肉芽腫と鼻細網肉腫あるいは悪性細網症として分けて考えられる傾向がある^1-5）。

　前者は自己免疫疾患と考えられ後2者は腫瘍あるいはそれに近いと考えられているがその病因についてはいまだ明らかでない。これら疾患はきわめて急激に進行する場合，現在のあらゆる治療法にも反応せずその病態はいわゆる悪性腫瘍以上に激烈である^6）7）8）。この様な激しい病態には腫瘍としてより炎症としての性格が関係している。細網内皮系の反応は強い外因を考えさせるが特異な感染微生物も認められていない^9）。この様な点から自己免疫疾患が疑われるがその抗原物質，抗体に関してもいまだ明らかでない。

　A study is made on the foci and the mode of progress in Wegener's granulomatosis and 24 other cases of related nasal pathologies. No particular focus can be pointed out in the nose where Wegener's granulomatosis, reticulum cell sarcoma, reticulosis or reticulohistiocytosis may have predilection of developing, only, that these tumors are found to be involving both nares simultaneously from their early stages of onset. And again, they are found to be involving the lungs and the middle ear cavity. The disease progress in Wegener's granulomatosis is particularly noted by an early appearance of renal, dermal and arthritic involvements.

　With reticulohistiocytosis there seems to be a gradual involvement of the nose, ocular fossa and the palate leading to necrotic degeneration of the parts and in the final stage of the disease, abdominal, oericardial and hepatic structures will be involved.
