声門下肉芽腫2症例に対するT-tube挿入治験—Wegener肉芽腫との関連について

Two Cases of Subglottic Stenosis undergoing T-tube Insertion: In Relation to Wegener's Granulomatosis

I．緒言

　1931年Klinger^1）が上気道の破壊性肉芽性病変，糸球体腎炎，脾肉芽腫，および血管炎などの所見を示した症例を報告したが，1936年Wegener^2）が肺症状を伴った上気道の壊死性肉芽性病変，糸球体腎炎，および全身性の血管炎などを三主徴とする病態を一つの症候群として唱えて以来，この病態はWegener肉芽腫症と呼ばれている。

　Wegener肉芽腫症は上気道の炎症症状をはじめとして漸次多彩な全身症状を現わす全身性疾患である。本症は最近有効な薬物療法によりその予後が次第に向上してきている反面，罹病期間の長期化により比較的稀であった症状や所見もしばしばみられるようになってきた。

In two cases of Wegener's granulomatosis, diag-nosed with clinical manifestations, the patients received a prolonged drug treatment, and suffered from dyspnea due to granulomatous stenosis of the subglottis.

A 15-year-old boy and a 26-year-old man pre-sented the duration of the symptoms before complication of subglottic stenosis was achieved, of 3 years and 6 months and about 2 years respectively.

Insertion of a T-tube to relieve dyspnea from subglottic stenosis was successful in Case 1 but unsuccessful in Case 2 to permit relapse. The T-tube insertion was proved as an effective means of relieving dyspnea due to subglottic stenosis as long as the period of insertion is individualized in each patient's condition.
