Wegener肉芽腫の病因・病態に関する研究（Ⅰ）—好中球の機能について

ETIOLOGY AND SYMPTOMS OF WEGENER'S GRANULOMA (I): ON THE FUNCTlON OF NEUTROCYTES

　Ⅰ．はじめに

　Wegener肉芽腫は上下気道（鼻・肺）の壊死性肉芽性病変，糸球体腎炎，全身の血管炎の三主徴をもつ疾患としてWegener（1936）^1）により報告され，以後多くの報告が認められるが，近年腎病変を伴わないが類似の気道の壊死性肉芽性変化をきたす予後不良の疾患も報告^2）3）されWegener肉芽腫およびその類似疾患の本態に関する再検討が望まれる様になつた。

　これら予後不良疾患に共通する初期の病態は気道の炎症性肉芽性変化であることが多いが，これら部位に見出される病原微生物はstaphylococcusaureusがもつとも多く他の細菌たとえば特殊な細菌（結核菌）あるいは梅毒スピロヘーターなどの証明もなく，また特異的なウイルスも認められていない^4）。

　A study was made on the chemotaxis of neutrocytes of the patients with Wegener's granuloma with necrotic granulomatous changes and those with similar diseases. The results were as follows:

　1) The chemotaxis of neutrocytes was much lower in the patients than in the normal control.

　2) There was no presence or increase of substance found in the patients'serum which prevented the chemotaxis of neutrocytes.

　3) No abnormality was found in the chemotactic factor production of the serum.

　4) The random mobility of neutrocytes of the patients was normal.

　The authors assumed that Wegener's granuloma would be a type of secondary leukocyte dysfunction.
