上顎癌の晩期再発症例

DELAYED RECURRENT MAXILLARY CANCER

　I．はじめに

　近年平均寿命の延長とともにわが耳鼻咽喉科領域においても，癌の根治治療後長年月をへて再発をみとめたり，種類の異なつた悪性腫瘍を重複する頻度が増え，その報告に接することが比較的多くなつてきた。

　今回上顎癌治療後約25年をへて術創内に肉腫様腫瘤の出現をみとめ，始め重複癌を思わせたが，詳細な病理組織学的検討の結果癌細胞の多様性にもとづくいわゆる癌肉腫であつた1例を経験したので，その概要を述べ，若干の考察を加えて報告する。

　A case of carcinosarcoma is reported.

　A man, aged 66, complained of slight hemorrhage from the site of previous operation that covered the ocular fossa and the wall of ethmoid sinus from which squamous cell carcinoma was removed surgically, 25 years previously.

　Tissue examination from the site of the lesion strongly suggested fibrosarcoma at first but sarcomatous degeneration of the squamous cells appeared to be so strong that a final diagnosis of so-called carcinosarcoma was made.
