進行性鼻壊疽（Wegener肉芽腫，限局型Wegener肉芽腫，悪性細網症，細網肉腫）の治療—長期生存症例と短期間死亡症例の比較検討より

CLINICAL MANAGEMENT OF WEGENER'S LYMPHO-MATOSIS

　I．はじめに

　進行性鼻壊疽（壊疽性鼻炎）は予後不良の鼻腔の壊死性肉芽性疾患に対して用いられた病名であるが近年治療法の進歩でその予後は必ずしも絶望的でなくなつた。しかしながら今日なおその死亡率は癌，肉腫，白血病に劣らぬきわめて悪性の疾患である。

　本疾患の治療において問題となる点の1つは，初期の病態が似ているにもかかわらずその経過とともに病態が変化して異なつた疾患に発展する例を含んでいることである^1）-10）。言い換えれば異なつた疾患が類似の病態を示し鑑別し難いため適切なる治療が遅れることである。それは初期における局所（主として鼻腔）の壊死性肉芽性変化の鑑別において組織診断が非特異的で難しいこと^5,6,11）および各種の検査成績が類似していることが原因である^{9）10）12）}。今日，種々の診断法開発への挑戦が行なわれているがその前途はまだ明るくない。

　A comparative study is made on the state of pathology and clinical manifestations of Wegener's granulomatosis and other related diseases which were observed for the period 10 years. The material of the study consisted of 5 cases of Wegener's granulomatosis that lasted more than 2 years and 4 cases of deaths that occurred early in the course of the disease.

　There are at present increased evidence that Wegener's granulomatosis could be adequately controled by administration of adrenal cortex hormone or cyclophosphamide. In such cases a relatively large doses should be given from the beginning of the treatment with a careful attention at the time of its withdrawal.

　The method of treatment should be chosen in accordance to state of the disease, whether the disease is localized or general.
