I．はじめに
鼻・副鼻腔と眼窩とは，解剖学的にきわめて密接な関係にあり，眼窩は耳側を除けばその全周を副鼻腔によつて取りかこまれている。特に篩骨洞および蝶形洞と眼窩との位置関係は臨床的に重要な意味をもつている。前部篩骨洞外壁は，薄い紙状板によつて眼窩に，後部篩骨洞と蝶形洞は，眼窩尖端部および視神経管に相対している。原疾患が副鼻腔にありながら，眼科的な訴えを主症状とする症例に遭遇するのは，われおれ低しばしば経験するところである。
最近，広範な骨破壊を伴つた副鼻腔原発癌であるにもかかわらず，鼻症状は軽微で，視力障害を初発症状とし，比較的長い経過をとり，ついには両側失明にいたつた，いわゆる眼窩尖端症候群（orbital apex syndrome）を呈した1剖検例を経験した。
A 81 year-old man with orbital apex syndrome, is reported. Radiological examination revealed an extensive bony destruction resulting from a carcinoma of one of the right paranasal sinuses, most likely the posterior ethmoid sinus. It should be noted that in this case the patient had not complained of nasal symptoms, but hesuffered from a total loss of vision. At post mortem serial histological sections of the ethmoid area were prepared in frontal plane. These sections included the optic nerve which showedneural degeneration. However, the nerve was not invaded by the tumor nor was it inflamed. The optic neuropathy occurred, probably, because of compression of the nerve by the tumor.
