扁桃摘出術における剥離と剥離子

DISSECTION IN TONSILLECTOMY AND SPECIALLY DEVISED DISSECTORS

　手術とくに耳鼻咽喉科の手術では手術器械が大きな意義を持つている。扁桃は手術のやりにくい場所であるだけに，それを補うために適応した器械が必要になつてくる。扁桃の手術はその部位が特殊なものであり，解剖学的関係が特異なだけにその手術器械も特殊な型に工夫されている。しかもその特殊な形の器械はある手術法の手技に合うように考案されたものである。したがつて，それらの器械を理解することはその扁桃手術法の手技を理解することになる。しかし実際にはある人の手術に用いられている器械がその術者の行なつている手技にかなつているものとは限らない。

　扁桃の手術を安全に確実に行なうためには，行なう手技と用いる器械との間には密接な関係があるが，一般にはこのような問題は取上げられていないのでここに意見を述べることにした。

The tonsillectomy procedure may be categorically devided into two types, namely first, removal of the tonsillar capsule by the use of a snare after the slight dissection of the peritonsillar tissues, and second, the tonsillar capsule was fully dissected from the peritonsillar tissues and the remaining inferior pole was removed with application on the snare.

In order to avoid postoperative bleeding the author prefers the second procedure.

For this purpose the author devised 3 types of sharp dissectors which were introduced here.
