前立腺剥離子による経尿道的前立腺剥離切除術について

TRANSURETHRAL ENUCLEATION OF BENIGN PROSTATIC HYPERPLASIA WITH THE HIRAOKA DETACHING BLADE: EXPERIENCE WITH 30 PATIENTS

　前立腺剥離子による経尿道的前立腺剥離切除術を30例に施行し，同術式の有用性を検討した。まず利点の第1は，凝固など変性のない前立腺の外科的被膜の内側はこの方法でしか内視鏡的に観察できない点にある。この被膜を確認することで，初心者は前立腺腺腫と外科的被膜の解剖学的位置関係を知り，一方熟練した術者は自分が通常行つているTURでどれだけの組織が切除されずに残つているかを知ることができる。第2の利点は多少の熟練はいるものの，外科的被膜を損傷することなく腺腫を完全に除去できることである。広く普及するには同じ端子で剥離，凝岡ができるなどの改良が望まれるが，現段階でも有用性は十分にあると思われる。

Transurethral enucleation of the prostate (TUE-P) with the Hiraoka detaching blade preceded by transurethral resection of the prostate (TUR-P) was performed in 30 benign prostatic hyperplasia patients and usefulness of this maneuver was discussed. First of all, there is no doubt that no other transurethral endoscopic instrument except the Hiraoka detaching blade can demonstrate uncompromised prostatic surgical capsule which is unaffected by cutting or coagulation. By observing this surgical capsule, beginner
