副鼻腔肉腫2症例とその文献的考察

SARCOMA OF THE NASAL SINUSES, A REPORT OF TWO CASES

　I．はじめに

　副鼻腔の悪性腫瘍といえば，その大部分のものが上顎癌で代表され，肉腫の発生は比較的少ない。そのうちでも前頭洞に原発した肉腫は非常に稀で，本邦文献では僅かに6例をみいだすにすぎない。一般に副鼻腔肉腫のうちでは上顎洞原発のものがもつとも多いが，その大半は細網肉腫であり一方，線維肉腫の発生頻度は比較的多いといわれているにもかかわらずその報告例は意外に少ない。私どもは最近珍しい前頭洞細網肉腫症例と多彩な肺転移組織像を示した上顎洞線維肉腫症例を経験したので，ここに報告するとともに本邦文献についての若干の統計的観察を試みた。

　Two cases of sarcoma of the nasal sinus are reported.

　The first case a boy, aged 16, first noted a tumor mass in the right frontal region which was gradually increasing in size and with tenderness. Histological examination revealed the presence of reticulum cell sarcoma originating from the frontal sinus. The patient was treated with a combined therapy of surgery, radiotherapy and chemotherapy (employing exodan and vinblastin). No recurrence has occurred during the period of ensuing 3 years since his discharge from the hospital.

　The second case a man, aged 30, complained of a progressive swelling of the left cheek. Biopsy examination showed the presence of a fibrosarcoma in the left maxilla. The patient was treated by radical removal of the maxillary bone which was later supplemented by irradiation and chemotherapies. The tumor repeatedly recured; 10 months later the patient died.
