鼻性病巣感染は，副鼻腔炎による腎炎の発生が実験的にも立証されているのにかかわらず，日常の診療においてそれほど注目されていない。病巣感染のFocusに副鼻腔を初めて入れたのはBillings（1912年）であるが，扁桃や歯牙病巣の陰にかくれている観がある。このことは，副鼻腔炎では感染を反復しても，二次疾患との病歴上の関連性が明らかてない場合が多く，また病巣診断が確立されていないことに基づいていると思われる。
しかし鼻性病巣感染は，歯周炎から進展する歯性上顎洞炎の問題をも含んでおり，重要な課題てある。したがつて私は，本症について教室で臨床的に観察した成績に文献的老察を加えて述べたいと思う。
The incidence of focal infection of nasal origin ranges from 1-3% in general. This might be explained by the fact that the nasal accessory sinuses are provided with ostium through which the intrasinus secretion is propelled outwardly by means of the ciliary action of the mucous membrane whose lymphatic supply is meager and the staphylococcic infections are the most common.
The maxillary and ethmoid sinuses play the major role in forming the seat of focal infection. Many cases involved with acute sinusitis while others are found to be chronic.
The changes that occur in the sinus mucous membrane may range from granulomatous to edematous and to cystic; only in rare cases fibrotic changes may be recognized.
The diagnosis of the maxillary sinus may be positively established by its puncture irrigation and urtra-shortwave provocation.
For the treatment of the systemic involvement the type of nasal infection should be thoroughly investigated.
