いわゆる原発性気管オツエーナの1例とその病態像に関する一考察

THE SO-CALLED "PRIMARY TRACHEAL OZENA"

　Ⅰ．緒言

　単独に声門下腔あるいは気管に痂皮を形成し，呼吸困難，嗄声などを呈する，いわゆる原発性喉頭気管オツエーナは，比較的まれな疾愚とされている。本邦では市原ら^10）の統計によると47例の報告例があるにすぎない。私どもも最近1症例を経験し，それを機会に文献を渉猟する機会を得た。ところが発表症例によつて経過が異なり，粘膜病変も一様でないことがわかつた。さらには急激な発症を示し，乳幼児にみられるいわゆる急性悪性喉頭気管炎と類似のものと思われる症例もあつた。これは，いわゆる"オツエーナ"によつて連想される粘膜の慢性萎縮性病変像とは相容れないものである。すなわち現在では異なる病理像をもつ疾患が痂皮形成ということのみで，いわゆる原発性喉頭気管オツエーナという診断名を付されているように思われる。

　しかしこの症候に対して，より的確な診断名が付せられるべきではなかろうか。著者らはこの点に着目し，症例報告を兼ねてその病態像についての考究を発表する次第である。もちろん多くの症例で，おそらくは緊急事態のため，粘膜の病理組織学的検索は行なわれていないので，あくまで推論の域を脱し得ないが，あえて諸賢のご参考に供し，ご批判を仰ぎたい。

　A case of "primary tracheal ozena" in 22 year-old female wes reported. "Primary tracheal ozena" can be divided into two groups of cases according to the clinical manifestations. The one is charactrerized by a sudden onset of tracheal stenosis following upper respiratory infection and is readily responsive to treatments with anitibiotics and steroids. The other may take a prolonged course with atrophic changes in the tracheal mucosa regardless of the type of treatments.

　From the bronchoscopic and smear test findings the former may be considered as fibrinous inflammation of the trachea and the bronchus.The author suggests that this type of "primary tracheal ozena" should be designated as stenosing fibrinous laryngotracheobronchitis.
