鼻・副鼻腔の早期癌とその転移例の検討

THE DIAGNOSIS OF NASAL SINUS CANCER IN THE EARLY STAGE AND THE RATE OF METASTASIS

　鼻・副鼻腔のいわゆる早期癌，または初期癌の定義は，臨床的にも病理学的にも明確に規定されてはおらず，またこれを規定することは，必ずしも容易ではないと思われる。しかし，鼻・副鼻腔の悪性腫瘍の治療には，できるだけ初期に発見することが望ましいことは，今さらいうまでもない．われわれは，最近慢性副鼻腔炎ならびに鼻茸の術後再発，上顎洞血瘤腫，あるいは，臨床症状やX線像で悪性腫瘍が疑われ，試験的にCaldwell-Luc氏術式により上顎洞や篩骨洞を開放し，摘出粘膜の組織診断で，初めて，洞内に限局した悪性腫瘍の存在を診断し得た例，または，鼻腔のみに限局した癌で，化学療法でほとんど治癒させた比較的早期の癌と思われる症例を経験したので，これらの症例について臨床的，病理組織学的に検討した結果を述べるとともに，上顎洞内に限局した癌腫で，比較的早期に発見されたにもかかわらず，その転移巣により死亡した症例について検討した結果についても，あわせて報告したい。

　Among 8 cases of the cancer of nasal sinus recognized in its early development 6 cases complained of epistaxis and pain in the cheek X-ray examination of these cases generally reveal a faint unilateral indefinite sinus shadow. In 6 cases out of the 8 definitive diagnosis of cancer was made by microscopic examination of the sinus mucous membrane removed at the time of the operation. With administration of Bleomycin intravenously, two of these cases made a complete recovery. The authors call attention to the fact that cancers occurring in this region appear to be highly capable of forming metastasis to other parts of the body.
