　Ⅰ．緒言

　耳鼻咽喉科領域における出血は，一般外科におけるものと異なり，結紮不能ないしは困難なものが多く，ことに手術の場合には骨出血や実質性出血が多いため，止血剤が非常に重要な役割をしめている。止血剤は大別して，血液凝固促進剤，血管強化剤，毛細血管収縮剤，栓球増多剤に区別されている。プレマリンは静注用エストロゲンであり，その止血機序については多くの研究と報告があるが，わが科領域における止血機構は，Schiffのいうように，血管周囲の基質である酸性粘液多糖類がプレマリン投与により増加ないし強化することによるものと考えられ，広義での血管強化剤に入れうるものと考えられる。われわれは当科における外来手術患者にプレマリンを使用し，その臨床効果を観察するとともに，使用群と対照群について，酸性粘液多糖類を組織化学的に比較観察を行なつた。

　As a hemostatic agent Premarin, an estrogen, was found to be highly effective in stopping the postoperative hemorrhage of adenotomy and tonsillectomy. In cases of turbinectomy premarin was administered as premedication in 10 cases while it was omitted in 5 other cases. In Premarin administered cases the presence of perivascular polysaccharids, which were determinded by special staining methods advocated by both, Reinhart and Sylven, was markedly increased whereas, in 5 other cases did not show any increase. The authors believe that, the presence of the increased acid polysaccharid as the mechanism of increased hemostasis.
