私の蝶形骨洞開放手術法—（Paranasal Method）（第1報）

MY METHOD ON THE OPERATION OF SPHENOID SINUS (PARANASAL METHOD)

　緒言

　蝶形骨洞手術については，西端教授に多くの著述があり，また浅井（1960）^1），後藤（1960）^2）の記載もあるが，ほかの副鼻洞手術にくらべると簡略である。日耳鼻総会の古城（1964）^3）の演説抄録を別として，論文としては，竹本（1960）^5）のものを挙げる。古城^3）は自然口より，竹本は篩骨洞底より蝶形骨洞に侵入している。私はparananal methodにより篩骨洞底と自然口の両方面より同時に蝶形骨洞を手術した。篩骨洞にたいするparanasal手術は1952年北村^5）により，前部篩骨洞手術についてという題名のもとに発表せられた。上記北村^5）の論文にparanasalという表現は使われていないが，その術式はparanasal methodである。1961年北村^6）ははじめてparanasalという言葉を用いた。paranasalの意味は手術方法の項をお読みになれば了解頂ける。私は北村式paranasal篩骨洞手術法を応用するとともに，蝶形骨洞手術に適するようこれに工夫を加え，1952年以来蝶形骨洞炎を117側手術した。以下にその方法をのべる。手術所見その他については第2報をもつてする。

　Niki reports his own method on operational opening of the sphenoid sinus as follows: The maxillary sinus and the posterior ethmoids are operated through oral approach. A portion of the frontal process of the maxillary bone is removed along with the air cells of the ethmoidal labyrinth. A portion of the perpendicular part of the palate bone is removed and its connection to the middle turbinate is separated. By pushing aside the freed middle turbinate the natural ostium of the sphenoid is further enlarged; the mucous membranes within are completely exenterated.
