　慢性中耳炎が少なくなつた今日慢性副鼻腔炎は我々臨床家の最も多く接する重要な疾患であり，37年度児童生徒の全国的統計を見ても中耳炎が0.5％近くになつているに反し，蓄膿症はまだ2％近くの高率を示している。

　衛生環境の進んだ米国やEEC諸国では既にこの疾患は少なくなつており，ウイーンと我国の鼻疾患を比較した奥田の報告^1）ではウイーンの慢性副鼻腔炎患者は我国の1/6にすぎない。私は当地方に特に多い線維工場に於て慢性副鼻腔炎患者の多発している事より，上顎洞病的粘膜を線維の研究に使用する偏光顕微鏡で観察し組織内に意外に多くの線維性塵埃の侵人している事を見出し，又線維工場以外の患者よりの病的組織よりも線維性塵埃を多く見出す事が出来たので慢性副鼻腔炎の原因の一つに塵埃のある事を提唱した^2）。

　The pathological mucous membranes obtained from chronic sinusitis which is the most prevalently met clinical cases in otolaryngological practice was examined under polarizing, microscope. It was surprising to find a large amount of dustparticle infiltration in these specimens. One reason which the sinusitis becoming chronic may be due to this dust-particle infiltration as a foreign body intrusion and the inevitable accompaiment of bacterial invasion. Japanese live on tatami (straw-mats) where dust and bacteria are bound to accumulate; the living quarters are usually over-crowded; sanitary conditions are poor and general concept towards house-dust is rather insensative. These factors may be the real reason in finding more chronic sinusitis morbidity among the Japanese as compared to the populace of either Ameica or Europe.
