耳鼻咽喉科手術とショツク

　はしがき

　手術の際，特別に原因と考えられることなく，突然，痙攣を起し意識不明となり，呼吸心臓いずれも停止し，短時間の内に死亡することがある。耳鼻咽喉科領域においても，たとえば，扁摘時の局所麻酔の際，あるいは手術際中に突然痙攣を起して死亡したり，又，扁桃周囲膿瘍で試験穿刺を行つただけで死亡することがある。又，単に少量の局所麻酔剤を顔面や頸部に注射したりするだけで，全身状態急変し，あらゆる努力の甲斐もなく死亡することもある。多くの場合，これら患者は術前には殆んど健康体であり，何故の急死か原因不明のことが多く，更に，剖見所見によつても，直接死因と目すべき形態的変化は認められないのが通常である。こうした事故を惹起すのは極めて多様な因子によつて条件づけられ，これらの因子のいくつかが影響し合つて症状を現わしてくるように思われる。私共は当教室およびその他2，3の耳鼻科教室，病院，ならびに東京都監察医務院において得られたシヨツク症例および過去の文献から集録したシヨツク症例を検討して，統計的にこれらの症状を観察し，さらにシヨツク死に対する若干の考察を加えてみたいと思う。

　Suzuki and associates attempted to make a study of shock that may be met with at the time of operation. Forty two cases of shock of this type are collected from various sources, Otolaryngological Department of Keio University as well as of that few other Universities and from the records of the Tokyo Metropolitan Medical Examiner Office

　These cases are studied from statistical, point of view; whether the cause of shock to be factors lying internal or external or, existence of relation if any between the onset of shock and stimulation of autonomic nervous system.
