ストマイ難聴に対するChondroitin sulfateの予防並びに治療効果

　まえがき

　Dihydrostreptomycinの使用により激増した所謂ストマイ難聴が無自覚性高音域聴力損失即ち潜在性聴力損失の発現を以てその初発症状とするものであることは私が昭和28年度日本耳鼻咽喉科学会総会に於て発表し，以後屡々強調してきた所であつて，現在では既に衆知の事実となつた。而してこの潜在性聴力損失の時期を超えて聴力損失が更に中音，低音域に迄波及し始めると，その聴力障碍は仮令Dihydrostreptomycinの投与を中止しても急速に進行する傾向を有し，聴力に対して予後甚だ不良の結果を招来するために，高度のストマイ難聴発現を未然に防止するには定期的聴力監視によつて潜在性聴力損失を検出し，以後のDihydrostreptomycinの投与を中止するのが最も確実安全な方法と考えられてきたのである。

　然しながら実際問題としては潜在性聴力損失を発見しても，結核性疾患の病状に鑑みDihydrostreptomycinによる治療を中断し得ない場合が多く，剰さえ潜在性聴力損失発見と同時にDihydrostreptomycinの投与を中止した場合に於てすらも聴力像が漸次増悪の一途を辿る例も相当数認められる状態である。これ即ちストマイ難聴の予防法の確立が，こゝ数年来殊に強く切望されてきた所以であつて，吾々の教室に於いても過去，Vitamin Aを始めとし各種のVitamin B₁製剤，Parotin, Neurotropin, BAL等の予防効果を検討してきたが，Vitamin A以外の他のものには，殆んど全く予防効力を証明することが出来ず，Vitamin Aの効力と雖も他のものより比較的良好と言い得る程度で，多くを期待し得ないことが明らかとなつた。これらの薬剤のストマイ難聴予防効力に関して，今迄に耳科学会に於て発表されれた諸家の報告もこれと大同小異であつて結論的に見て何れにも著明な予防効力の無いことを知り得るのである。

　Shida reports that, for the prevention and treatment of hearing loss that may be sustained by streptomycin therapy, particularly by use of dihydrostreptomycin, clinical use of chondroitin sulfate has shown results that are far superior than either vitamin B₁ or A. Clinical application of this agent has been made after attaining results by extensive animal experimentations which was previously reported in The Journal of the Oto-Rhino-Laryngological Society of Japan, No.2, Vol. 60. When treatment with chondroitin sulfate is instituted in cases where hearing loss is established in not a too long a period, that is, with a few months of its onset, a complete recovery or a marked improvement of that loss are generally the rule. Establishment of this fact appears to be important in that, heretofore, no treatment was available for the conditions referred to.
