扁摘後疼痛の成因と緩解について—第二報 自律神経系要素

　Ⅰ．緒言

　扁桃摘出後の疼痛の有無は色々の意味で，重大なる意義がある。鎮痛方法には古来より各種の薬品が用いられるが，何れの鎮痛剤も血管壁の弛綾，血液成分の変化を生じ，多少とも，出血を招来し易い事は明かである。依つて，本院では先年来，其の疼痛の成因を究明し，其の結果無害の方法を考按，之を実施し，良好な結果をあげて居る。其の第一報として「VB₁欠乏」に就いて本誌25巻12号に発表した。

　当時，扁摘後疼痛には，嚥下時にのみに見られる嚥下痛と，嚥下其の他に関係なく何時間も継続して生ずる強烈な自発的咽頭痛とあり，前者は局所麻酔の減退と共に現われるもので扁摘例の殆んど凡べてに見られるものである。之は嚥下作用に際しての，接触，筋攣縮等の器械的刺戟が，創傷の知覚神経に与へられる都度感ずるもので，前回と同様に，今回も此の疼痛に就いての，研究は除外し，後者の自発的咽頭痛に就いてのみ研究した。前回此の咽頭痛は，全扁摘例の約半数に認められ，疼痛の性質及びMester反応陽性Eosinophilie並びに鎮痛効果の点より，斉藤教授の（京大）B₁欠乏性「ロイマ」＋xであろうとし，其のxは，VB₁による鎮痛不可能の症例があるためと，VB₁100mgと言う大量使用に依つても鎮痛所要時間を短縮出来ないから，何か他に疼痛発生の要素があるものとし，此等の不明の要素をと仮定したのである。而して，今回は此のxの内に自律神経系の要素が含まれて居るか，否かを検索した。然し従来の学説の如く自律神経を以て遠心性機構と考えれば不合理であるが，現在では何等かの関連性があると知られて居るので，其の検査結果を基礎とし，Eppinger u. Hessの仮説を信じて治療を考按し，良好な結果を得たので，此処に発表して，諸賢の御追試を乞う次第である。

　KAMIO and associates maintained in their previous report elsewhere that pain the after-effects of tonsillectomy may be divided into two types, one that may be elicited during the act of deglutition and the other that mey be dominant as spontaneous pharyngeal pain. Particularly the latter in their opinion is caused by a condition which may be paraphrased as the presence of defficiency factor of Vitamin B₁ plus X (an unknown factor). In the present study the authors find that with those in whom complaints of spontaneous pain are made are particularly those in whom their parasympathetic system is found to be overly stimulated or their sympatheitc system as a whole to be in a state of imbalance. Such pains may be adequately eliminated by use of adrenalin alone and/or in combination with Vitamin B₁.
