試作せる上顎洞穿刺用鑿について

　上顎洞穿刺に際し，しばしば難渋するのは，下鼻道の骨壁がかたいときである。私はかつて岡山に於ける日本耳鼻咽喉科学会中国地方会の席上，かたい骨壁をもつた下鼻道側壁を穿刺しようとしてSchmidt氏穿刺針が折れ，折れ端が不幸にも左気管支におちこんだ症例を追加した。レ線検査でたしかめたのち（第1図），患者の上半身を寝台の端より垂直にぶら下げることによつて折損片は難なく口外にとび出し事なきをえたのは不幸中の幸であつた。これにこりてその後は穿刺の際は患者の頭を．深く下げ念には念をいれてやることにしていたのに，またまた骨壁のややかたい症例で穿刺針が折れ，今度は胃の中に落ちこみ，これもレ線撮影（第2図）で見つけて一安心，2日後肛門より無事排出された。このことあつて以来，多少とも骨壁がかたいと思われる場合は，まずStacke氏のみで下鼻道骨壁をたたきあけ，しかるのち穿刺洗滌することに決めた。

　しかしのみで下鼻道をたたきあけることは患者にかなりの苦痛を与え，出血も多い上に，下甲介が腫大して下鼻道がせまい場合はのみがうまく入らないといつた難点がある。更に都合のわるいことは折角のみで穿孔させても，その穴に丁度Schmidt針をはめこむことが，両者の彎曲度のちがいのために，必ずしも簡単でないことがある。

By use of the chisel under discussion in order to pierce the hand bony wall a few taps on its head would acco-mplish the desired results which, thereaster, allow insertion of a canula either diagnostic or irrigational purposes with ease.
