副鼻腔炎と其の頭蓋内侵襲に就て

　副鼻洞炎から又其の手術後に頭蓋内合併症を起す事は比較的稀で之には云う迄もなく手術に關係なく起るものと手術に關連して生するものとある．私は以前から副鼻洞手術の際の臨床解剖學に興味を持ち之に關しての數篇の論文を報告してあるがこの事が幾分は役立つて手術時脳膜を露出したが頭蓋内侵襲を避け得た2症例と，副鼻洞炎の所謂Manifest formの2例に於て兩側共頭蓋内合併症の危險が濃厚であつたが治癒した症例に遭遇したので此處に報告すると共にペ劑使用と之等手術との關係等に就ても觸れて見たい。

TAKAHASHI In operating upon two cases of mutiple sinusitis, of which there were no subjective complai-nts Takahashi finds, in one case, developmental abo-normality of ethmoidal cells of which the cerebral wall when exposed showed dural pulsation. In the other, which showed by roentgenogram examinatively a dehiscence of considerable size in the posterior w-all of sphenoidal sinus exposure and protr&ision of a portion of the pituitary body and visibleness of pul-sation of internal carotid artery when that part of the wall was attained but, no seepage of cerebral flu-id was seen. In both' cases operative wounds were packed with gauze saturated with penicillin and the penicillin was also administered parenterally. The recovery was uneventful in the two cases.
