孤立性鼻咽腔ポリープの摘出法—（笹木氏変法）

　緒言

　鼻咽腔に存在する孤立性ポリープの摘出法に就て笹木教授は，（1）ネラトン氏カテーテルを一側鼻腔より腫瘍莖部の下方を通じて口腔に出し，之を絹糸と交換し，（2）同じカテーテルを同側鼻腔より今度は腫瘍莖部の上方から口腔に出し，（3）口外で絹糸とカテーテルを結び，カテーテルを鼻腔から拔く―と云う操作で絹糸（次で絞断線）を一側鼻腔を通して腫瘍莖部にかける方法を述べられた．

　我々はたまたまこの笹木氏法の適應と思われる1症例に遭遇し同法に依る摘出を試みたが，腫瘍の位置の関係が如何にしても（2）の操作に成功せす，止むを得ず次に述べる樣な方法を工夫し実施した所，比較的簡單に摘出する事が出來た．根本的に笹木氏法と異るものではなく，しかも氏の原法に比して操作がやゝ入組んではいるが，腫瘍の位置や大きさ等に依りこの方がやり易い症例もある事と思うので，借越ながら笹木氏の1変法として発表させていただく事にする．

Suzuki and Oguchi describe a new method for removing isolat ed nasopharyngeal polylap, the steps of which closely follow principles that has been advanced by Sasaki. Essential point of difference, however, lies in making use of both nasal cavities in manipulation of Nelaton's catheter which is inserted on one side and brought out on the other to create a loop at the nasopharynx; the loop thus formed is slipped over the polyp onto its neck from below upwards. These steps of this method are somewhat more complicated in comparison than to those which described originally by Sasaki but, it may be appreciated for disposal of a polyp which might be closely attachcd to the roof of nasopharynx with relatively short neck.
