原田氏病における皮膚科的所見

DERMATOLOGIC SYMPTOMS IN HARADA'S DISEASE

I．緒言

　原田氏病，Vogt—小柳氏病は非外傷性に，交感性眼炎は外傷後に発生する重篤なる葡萄膜炎で，何れも眼病変と共に白斑，白毛を来すことが知られており，非外傷性の葡萄膜炎の中，脈絡膜炎の症状を主とするものを原田氏病，虹彩毛様体炎の症状を主とするものをVogt—小柳氏病と呼ばれている。原田氏病，Vogt—小柳氏病共に，初期には頭痛，嘔吐，発熱等の脳膜症状がみられ，眼症状発生と共に，難聴，白斑，白毛，脱毛等の合併症状を来すが，これらは必ずしも必発の症状でなく，殊に白斑，白毛，脱毛の三つの皮膚症状を伴つた例は比較的稀である。1906年Vogt^1）は非外傷性の葡萄膜炎に白斑と白毛を併発することを記載し，1929年小柳^2）は，更に脱毛と難聴を加えて1症候群とした。また，原田^3）は1926年に網膜剥離を伴う急性瀰漫性脈絡膜炎について報告している。その後，眼科方面よりの報告は比較的多いが，皮膚科方面よりの報告は少い。

　私共は最近，白斑，脱毛及び白毛を併発した原田氏病の1例を経験したので，ここに報告する。

　A 46-year-old male patient lost his eyesight suddenly, and he was made a diagnosis of Harada's disease (Chorioiditis diffusa acuta) in the ophthalmolgic clinic of Osaka City University. Four months later, incomplete depigmented macules appeared on the back, loin, back of hands, extensor surface of right arm, etc. At the same time, some patches of incomplete hair loss with depigmentation developed in the occipital region. They resembled alopecia syphilitica in a clinical appearance. In the histolgical findings of a depigm nted area, malaningranules decreased in the epidermis,and enzymatic activity of melanocytes was weakly maintained in the basal layer. Following the treatment withoral administration and eyelotion of corticosteroid hormones, ophthalmologic symptoms were improved, and slight growth of black hairs was found in the patches of hair loss in the occipital region. Some depigmented areas cleared, and new areas developed, as if the depigmented areas were wandering.
